Let’s Make this Easy! Below you will find all of the necessary documents and pet requirements you will need to complete prior to using any of our services here at Muddy Paws Country Spa. Please be aware that dogs participating in daycare have 2 additional requirements.

1. Completed New Client Form

2. Signed Liability Release

3. Pet(s) are current on all vaccines

Dog: Rabies, Bordetella, and DHPP

Cats:Rabies, FVCRP, and Feline Leukemia

Additional Requirements for Daycare Dogs

1. Dogs Participating in our daycare groups must be spayed or neutered after 6 months of age.

2. Dogs Participating in our daycare groups must pass a daycare evaluation. This means that all dogs must prove to be friendly and social and maintain passing behavior.
